Interprofessional Program Development for a Network of Family Health Teams
The Academic Family Health Team (AFHT) Forum is an alliance of primary care practices affiliated with the University of Toronto. Forum members faced common challenges in the evolution to Family Health Teams and integrated team practice approach. Challenges ranged from the mechanics of program design to the incorporation of teaching within the newly formed interprofessional care models. The AFHT Forum successfully collaborated around start up issues such as governance models and infrastructure planning. Thus there was a strong interest in continuing to harness the collective resources and wisdom of the fourteen members and put it into joint program design activities. This collaborative effort would result in a more comprehensive approach to program design than could be done be each organization on their own.

With phase 2 funding through the Interprofessional Care and Education Fund (Healthforce Ontario) the Forum is undertaking program development in two additional areas, namely the management and prevention of childhood obesity, and managing the complex diabetes patient.

A key success factor has been to initiate the work from an evidence base. Task Groups reviewed clinical guidelines and information on team based care, provided by the Guidelines Advisory Committee. From this base, Task Group members designed an optimal system for team management of patients in the targeted care areas. Protocols, pathways, tools and other resource documents were developed by the Task Groups, and piloted in participant organizations. The protocols and tools are designed for local/individual organization adaptability.

The Healthforce Ontario funding provides for the team protocols to be translated into training modules for in-service delivery to ten FHTs across the province. Delivery of these modules will be led by health professional champions from various disciplines, involved in the creation of the materials.

Request in-service session at your organization
The Healthforce Ontario funding provides for the team protocols to be translated into training modules for ten in-service sessions to FHTs across the province. Delivery of these modules will be led by health professional champions from various disciplines, involved in the creation of the materials, thus reinforcing the value of interprofessional collaboration for care delivery.
